Why results differ

Same shoulder, same filler, yet the result differs from clinic to clinic.

In one place you get a sharp, squared shoulder. In another it turns dull and blunt. The difference is not the product. It is the rules.

Body filler runs on completely different rules from facial filler. The moment filler goes into the body, the volume moves up a class, and with it the safety standards, the product, the instruments and even the position all have to change. The gap between a clinic that knows these rules and one that simply does what it does for faces becomes the gap in the line.

The face stays around 2 to 10 cc. The body takes 15 to 20 cc for the shoulders alone, and close to 40 cc for the hips. It is the difference between a cup of coffee and a whole pot. Just as the same beans are handled completely differently, this difference in volume creates every principle below.

Safety comes before a pretty line.

The first thing we look at in body filler is not the line. It is safety, because the volumes are large.

A low cross-linker (BDDE) product

Filler uses a cross-linker to bind hyaluronic acid so that it lasts. On the face, where the amount is small, the cross-linker content often does not need much attention. The body is different: 10 cc or more goes in at once. With a high cross-linker product, the risk of foreign-body reaction and inflammation rises in proportion to the volume. That is why a dedicated low-BDDE body product is the baseline. Putting a facial product into the body in large amounts goes against the principle.

A low lidocaine (anaesthetic) product

Most fillers on the market contain lidocaine, an anaesthetic, to reduce pain. In the small amounts used on the face that is not a problem. At body volumes, however, the total lidocaine can approach a dangerous level. The shoulders (15 to 20 cc) leave little margin, and the hips (40 cc and more) are clearly risky. So for the body we choose products with low lidocaine content, and manage pain separately with tumescent anaesthesia, a local anaesthetic that swells the treated area.

This is where skill in body filler is decided. Before "how beautifully" comes "how safely can you handle large volumes". That is why we brought in a dedicated body filler ourselves. We do not repurpose facial products for the body, and we design the anaesthesia protocol separately to match the volume of each area.

Not a little here and a little there. Designed like an implant.

Facial filler is usually distributed gently, "a little here, a little there". The body calls for a different way of thinking altogether. We approach it almost exactly as we would an implant. The concept behind shoulder implant surgery, pushing out the outermost point to create a squared line, applies directly to shoulder filler.

That is why body sculpting is eighty percent design. Decide the silhouette you want, approach it as filling that shape, and only then does the result hold up. The moment it is priced "per cc", it stops being body sculpting and becomes volume filling. In consultation we draw the target silhouette together first, then begin. That is the decisive difference from a clinic that starts with "how many cc would you like".

A treatment this hands-on is not for just anyone.

Body filler is a treatment where the right answer cannot be seen. Even on the same shoulder, where and how much to lift for a beautiful result cannot be measured with a ruler. In the end the practitioner's aesthetic sense decides the result outright. It is, in other words, a treatment that depends on the hands.

That is why we do not hand body treatments to just anyone.

Before creating the shape, we clear what is in the way.

This matters enormously, yet it is surprisingly often overlooked. However beautiful the shape you create with filler, if thick fat covers it the line will not show. Even good filler placed exactly right gets its contour smudged by the fat on top.

The shoulders show this most starkly. For someone with thick fat over the shoulder muscle, filler alone never produces a squared shoulder, only a persistently dull silhouette. Collarbones that do not show are often hidden not by small bones but by the fat above and below them. Keeping the order right changes the result dramatically.

That is why we never recommend filler first. If your fat needs clearing first, we tell you that order honestly. Selling filler alone is easier for the moment, but if the result never comes, both sides lose.

It often happens that a shape sculpted perfectly while lying down turns into a completely different silhouette on standing. Only when it is shaped in the real position, with gravity at work, does the figure you see in daily life come out right. So we treat seated, looking into the mirror together with you. Not a line that satisfies only the person making it, but the line you will actually see.

The shoulders make this instantly clear. Wanting a squared shoulder, you have filler placed at the tip to set the angle. But if the trapezius, the muscle between shoulder and neck, is knotted and bulging, the angle you worked for will not show, because the shoulder line is pulled upwards. Place toxin in the trapezius and the knotted muscle releases, the shoulder line drops and the neck looks longer. Only then does the squared line made with filler come alive. One adds, one subtracts, and together they complete a single silhouette.

This is the combination often called "wedding dress Botox", sought by many brides for the shoulder and neck line a dress reveals.

Toxin, too, separates the skilled from the rest. Placing it anywhere in the muscle and placing it at the exact point where the muscle's nerves gather (the motor point) give completely different results. Hit the exact point and even a small dose works clearly and lasts. So rather than pushing the units (dose) up blindly, we find the exact spot and place it there. This is why we never talk only about filler when we consult on shoulders. We design it as a set from the start: where to fill (filler) and where to take away (toxin) so that the line comes alive.

Once the shape is set with filler, one last step remains: the firmness and texture of the skin. However beautifully the line is set, if the skin covering it is loose and thin, the finish falls short. Areas where weight was lost or fat was cleared are especially prone to leftover skin and disappointing firmness. The shape is there, but the surface has not caught up.

FAQ

Body filler: what people ask before a consultation.

+Do you use the same product as for facial filler?

No. Because 10 cc or more goes into the body at once, we use a dedicated body filler with low cross-linker and low lidocaine content. We do not repurpose facial products for the body, and we design the anaesthesia protocol separately to match the volume of each area.

+How is it anaesthetised? Does it hurt much?

Pain is managed with tumescent anaesthesia, a local anaesthetic that swells the treated area, so that we do not rely on the lidocaine mixed into the filler. Pain sensitivity varies from person to person, so we do not promise you will feel nothing, but we check in with you as we go.

+How many cc go in? Is the cost calculated per cc?

It depends on the area and your build. The shoulders take around 15 to 20 cc, and the hips sometimes close to 40 cc. But rather than starting from "how many cc", we draw the target silhouette first and then set the amount needed to fill that shape. The exact volume and cost are explained in consultation, in front of the mirror together.

+I carry quite a lot of fat. Can I have filler first?

We do not recommend it. When thick fat covers the contour, the line turns dull however well the filler is placed. In that case clearing the fat comes first and filler second. If your fat needs clearing first, we tell you that order honestly.

+Do I have to have trapezius toxin at the same time?

Not everyone needs it. When the trapezius is knotted and raised so that the shoulder line is pulled upwards, filler alone cannot produce a squared shoulder, so we design toxin as its pair. Only when it is needed, and placed at the motor point where the muscle's nerves gather, in a small dose.

+How is Body Sculptra different from filler?

Where filler creates shape with immediate volume, Body Sculptra uses PLLA to prompt your own body to produce collagen, building skin thickness and firmness. It changes gradually over 3 to 6 months and looks natural because it is your own collagen. We use it at the final stage, finishing the surface once the shape is set.
